Oak Creek Canyon, AZ – A toddler was killed and two other people were injured Sunday morning in an off-road vehicle accident near the top of Oak Creek Canyon, according to the authorities.

The deadly accident happened just before 11:00 a.m. on June 22, in the area of Forest Road 535 and State Route 89A.

Authorities said an off-highway vehicle (OHV) driven by 37-year-old Miguel A. Pacheco rolled over.

A 19-month-old in the vehicle was ejected and was pronounced dead at the scene. The toddler was not in a child safety seat or a suitable restraint.

A 7-year-old and a 17-year-old in the vehicle sustained non-life-threatening injuries.

Pacheco was arrested and booked into jail on charges including manslaughter, three counts of aggravated assault, two counts of felony endangerment, two counts of aggravated DUI, reckless driving, driving on a suspended/revoked license and possession of narcotics. He was being held on a $500,000 cash-only bond.

No other details were provided.

The authorities are investigating the accident.
