Lewiston, IL. – Melissa Phelps, 42, of Havana, has been identified as the woman who died in a car accident on Monday in Lewiston, Fulton County.
The fatal collision happened around 12:03 p.m. on U.S. Route 136 near Bottom Lane, according to the Illinois State Police.
Phelps was driving a car when she collided with a commercial vehicle in unclear circumstances.
Phelps was killed in the accident.
