Deputies said an elderly woman made an unsafe lane change and caused a box truck to lose control and cross into oncoming traffic where it hit two other cars.

Glendale, AZ. – A man was killed after a chain-reaction accident on Monday afternoon in Glendale, according to the Glendale Police Department.
The accident happened a while before 2:30 p.m. on May 18, near 67th Avenue and Utopia Road.
Deputies said an elderly woman made an unsafe lane change and caused a box truck to lose control and cross into oncoming traffic where it hit two other cars.
One of the motorists of the vehicles hit by the truck was declared dead at the crash site. He was the only occupant of the vehicle.
Two other drivers involved in the collision were taken to area hospitals for treatment of non-life threatening injuries.
No additional information concerning this crash was released.
The incident remains under investigation.
