Holbrook, AZ – The Arizona Department of Public Safety on Wednesday identified the victims killed in a multi-vehicle crash on Interstate 40 in northern Arizona last week.
DPS identified victims as:
- Sarah Broyel-Case, 50, of Fort Collins, Colorado
- Jeet Vikram, 34, of Ozone Park, New York
- Sophia Case, 17, of Fort Collins, Colorado
- Adam Mychajliw, 17, of Scottsdale, Arizona
- Samuel Case, 51, of Fort Collins, Colorado
Authorities said the deadly chain-reaction happened at about 1:30 p.m. on Friday, March 14 in the westbound lanes of I-40 near milepost 294, approximately 10 miles east of Holbrook.
According to Investigators, a commercial truck tractor and trailer rear-ended a Cadillac Escalade. The impact caused a chain-reaction collision involving a Jeep Wrangler and another commercial truck tractor and trailer.
Four people were killed in the crash. A fifth person, later identified as Samuel Case, died at the hospital on Tuesday.
Reportedly, Mychajliw was a senior at Saguaro High School and a member of the cross-country team.
Two other people were injured in the in the incident.

Holbrook, AZ – The authorities are investigating a deadly multi-vehicle collision that took place near Holbrook Friday afternoon.
According to the Arizona Department of Public Safety, the accident happened at around 1:30 p.m. on westbound Interstate 40 near milepost 294.
The authorities reported that multiple vehicles collided and caught fire in an incident believed to have been weather-related.
DPS said 5 people were killed in the crash.
Authorities blocked the westbound lanes of I-40 as they worked to clear the scene.
The cause of the incident is under investigation
