Jesse Forman from Chino Valley was driving a Toyota 4-Runner and was with his three kids and the mother of the children on Williamson Valley Road near West Levie Lane at about 3 p.m. on March 22.

Prescott, AZ – Three people were killed and several others were injured in a car accident that took place in Prescott on Saturday afternoon, according to the Central Arizona Fire and Medical Authority.
The deadly collision happened on Williamson Valley Road near West Levie Lane at about 3 p.m. on March 22.
Investigators said believe the driver of a Toyota 4-Runner crossed over double lines and collided head-on with a Jeep SUV.
The Toyota driver was driving with his wife and their three young children. The Jeep was operated by a woman in her 60s and was with her adult son.
The driver of the 4-Runner and one of the children died at the scene. Another child was pronounced dead while on the way to the hospital via an ambulance.
Four patients, including a child, were airlifted to hospital with serious injuries.
Both sides of Williamson Valley Road were shut down as authorities investigated the accident.
The incident remained under investigation.
