The Oregon State Police said a white Freightliner semi-truck, operated by Joseph Jack Botelho, 64, of Veneta, was traveling westbound on SR 126 when it collided head-on with an eastbound white Ford T-350 van operated by Mathew Decker.

Lane County, OR – Mathew Jacob Decker, 37, of Eugene, has been identified as the man killed in a head-on crash on Highway 126 in Lane County early Thursday last week, according to the Oregon State Police.
Authorities said the deadly crash happened at about 2:23 a.m. on April 3 on State Route 126 near milepost 6.
The Oregon State Police said a white Freightliner semi-truck, operated by Joseph Jack Botelho, 64, of Veneta, was traveling westbound on SR 126 when it collided head-on with an eastbound white Ford T-350 van operated by Mathew Decker.
Decker suffered fatal injuries and was pronounced dead at the scene.
Botelho sustained minor injuries and was not hospitalized.
Authorities said the highway was shut down for approximately five hours during the on-scene investigation.
The incident remains under investigation.
